MRI-ultrasound fusion-guided prostate biopsy and TULSA-PRO®

At UIC, we believe that the future of prostate care lies in precision medicine—delivering targeted, individualized treatments that maximize outcomes and minimize side effects. To achieve this, we’ve brought together two of the most advanced technologies in urologic care: MRI-ultrasound fusion-guided biopsy and the TULSA-PRO® transurethral ultrasound ablation system. Together, these technologies allow us to seamlessly bridge the gap between accurate diagnosis and precise, non-invasive treatment.

Traditional prostate biopsies rely on a systematic, grid-based approach, often missing clinically significant cancers—particularly those in the anterior or apical regions—and sometimes leading to repeated procedures, anxiety, and delayed care. MRI-ultrasound fusion-guided biopsy revolutionizes this process. First, a multi-parametric MRI is performed, capturing detailed images of the prostate, including functional information about tissue density, blood flow, and cellular architecture. These images are then fused in real time with ultrasound guidance during the biopsy procedure, allowing us to precisely target and sample even small or otherwise hard-to-reach lesions. This approach not only improves diagnostic accuracy, but also helps differentiate aggressive tumors from indolent disease, supporting better treatment decisions and avoiding overtreatment.

Once a cancer diagnosis is established—or in the case of benign prostatic hyperplasia, once a treatment zone is determined—we can offer patients the TULSA-PRO® procedure, which represents a leap forward in minimally invasive prostate therapy. Unlike surgery or radiation, TULSA-PRO uses directional, high-intensity ultrasound delivered through a transurethral applicator, guided in real time by MRI. The technology allows us to “paint” a treatment area with millimeter-level precision, heating and destroying only the targeted tissue while sparing critical structures such as the neurovascular bundles, urinary sphincter, and rectal wall. Continuous temperature monitoring and active cooling systems ensure tissue safety and control throughout the procedure. Because there are no incisions and no ionizing radiation, the risk of incontinence, erectile dysfunction, and rectal injury is dramatically reduced.

MRI-ultrasound fusion-guided biopsy provides detailed maps of cancer location, volume, and aggressiveness, which we then use to develop a highly personalized TULSA treatment plan. Whether our goal is focal therapy—targeting just the tumor and leaving the rest of the gland intact—or whole-gland ablation, we can tailor every aspect of care to the patient's specific anatomy, disease profile, and quality-of-life goals.
